New Patients
New patients should bring the following items to their first visit and allow for at least an hour for us to go through your medical history and teach you how to use the patient portal system.
NOTE: If taking controlled substances, you will first see a provider to discuss treatment options and to determine if Bowdon Family Medicine will be beneficial to you and your healthcare needs. We will be glad to manage your medical needs, but we are NOT a pain management clinic. We will be happy to refer you to one.

(Completing these forms will decrease your initial wait time. If possible, bring the completed forms to our office the day prior to your visit.)
